The CUDOS (Clinically Useful Depression Outcome Scale) form below is a very useful tool to determine where you are in the depressive cycle. I fill this out prior to seeing Dr. Smith once a month (or every six weeks if I’m in a good spot). The exercise has become an important part of my recovery, because, by my score, I can gauge just how depressed I am and take the necessary course of action. To read more about this form, click here.
